The Psychotherapy Assessment Checklist (PAC) Forms allow therapists to gather a great deal of information and screen it rapidly. Patients fill out information about:

  • presenting problems
  • life history overview
  • DSM-IV diagnoses on all five axes

The PAC forms are free for your use with your patients. The following PAC materials can be downloaded in two formats (see below):

  • The Instruction Manual is a supplementary article to Treating Affect Phobia, which describes the PAC Forms and how they can be used.
  • The PAC Follow-Up forms can be used to assess therapeutic change after termination.
  • The PAC Forms themselves (7 pages) are available in English, Norwegian (translated by Elisabeth Schanche), and Spanish (translated by Enrique de-Urquiza). The Follow-Up forms are available in English and Norwegian.
  • The one-page PAC Summary Form allows the therapist to summarize the patient's information from the PAC Forms.
  • The Sample Patient Letter can be edited, and then mailed to patients with the PAC Forms. The "html" button allows you to see the sample as a web page .

A note on file formats: "pdf" files allow documents to be viewed in ways that preserve the formatting; if you are having trouble with these links you may need to download the Adobe Acrobat Reader (click here for a free download). The "rtf" files can be opened in Microsoft Word or other word processors.
